At Benchmark Behavioral Health, we are happy to partner with other behavioral health professionals to help your clients get the support they need. We welcome referrals and will work with you throughout the admissions process to ensure consistent care for your patient. Our Admissions Coordinator will ask each referrer to complete a clinical packet, which should include the following information:

• A recent psychiatric evaluation
• Documentation of any psychological testing
• A treatment plan, treatment notes from current placement or discharge summaries from prior treatment
• Documentation of history of treatment with dates. (IOP, PHP, outpatient, previous RTC, ect.)
• List of past and current medications
• Legal history, current charges, probation information (if applicable)
• Documentation regarding any medical issues
• Medicaid card, insurance information
• Individualized Education Plans, (IEPs)
• Any additional information that would help our clinical team determine eligibility.

Our team will review the information and send you a determination. The referring party will be notified of acceptance or denial if the individual is not appropriate for our program.
